20. Requirements for Registration of Design:
(1). Colour label, photographs, diagram of arts, get up of any container, picture or packet or carton (3 inches x 3 inches size) on plain paper
(2) CD of Application of Design with Photographs of Design
(3) Affidavit of Applicant on stamp of Tk. 500/- (should be notarized)
(4) Name of Applicant
(5) Designation of Applicant-Proprietor / Partners / Managing Partner / Managing Director / Chairman / Secretary/ Chief Executive / President / Vice-President.
(6) Nationality of Applicant
(7) Name of Farm / Company / Association / Organization / Society / Club / Foundation / Partnership Farm.
(8) Address of Farm / Company / Association / Organization / Society / Club / Foundation / Partnership Farm.
(9) Description of Design- Description of Label / Container / Packet / Carton / Product whose design will be shown in the application of design
(10) Class of Design.
(11) P/A (Power of Attorney) Form-31 with stamp Tk. 1000/- for each case.
